What Is It?
Use this form to request lockout assistance from Campus Safety if you are locked out of:
- Your residence hall room or campus housing
- An academic or administrative building/office
- Another authorized on‑campus space you should have access to
Campus Safety will verify your identity and authorization for the space, then dispatch an officer to assist with entry.
This service is for individual access issues. It is not used for campus‑wide security procedures such as building lockouts or lockdowns, which follow separate emergency protocols.
If you believe there is an immediate threat to your safety or a crime in progress, call Campus Safety at 610-409-3333 or 911 instead of, or in addition to, submitting this form.
After you submit this request:
- Campus Safety dispatch will review your information and assign an officer.
- Response times may vary depending on call volume and officer availability, but lockouts are generally treated as high‑priority service calls.
Who Can Use This Service
- Ursinus students, faculty and staff
- Campus Safety staff submitting a lockout request on someone’s behalf (based on phone/radio contact)
When to Use This Service
Use this service when:
- You are physically locked out of a campus room/building where you believe you have authorized access.
- You have already tried your usual access method (key, card, code) and it is not working.
Do not use this service when:
- You are reporting a lost or stolen ID/key (that may be a separate service).
- There is an active threat or crime in progress → Call Campus Safety / 911.
