Property Damage Report

What Is It?

Use this form to report damage to university property so it can be documented, assessed, and repaired. Timely reporting of property damage supports safety, risk management, and appropriate follow‑up with Facilities, IT, and Finance (including insurance when needed).

Examples of property damage include:

  • Broken windows, doors, locks, or furniture
  • Damaged walls, ceilings, or floors (holes, graffiti, water damage)
  • Damage to university vehicles, carts, or equipment
  • Significant damage to lab equipment, AV/IT equipment, or classroom fixtures
  • Vandalism or intentional destruction of university property

This form is for non‑emergency property damage.
If there is an immediate risk to safety (e.g., structural damage, major water leak, live wires, gas smell, or damage related to fire or explosion), call Campus Safety at 610-409-3333 or 911 first, then submit this report if appropriate.

After you submit this form:

  • Campus Safety and/or Facilities will review the report.
  • If needed, a maintenance or repair work order will be created.
  • The incident will be logged for trend analysis, risk assessment, and when applicable insurance and financial reporting.

If you were involved in causing the damage, you are encouraged to report it honestly; this helps the college address the situation appropriately.

  • Students
  • Faculty and staff
  • Campus Safety / Facilities staff entering reports based on patrols, responses, or third‑party notifications

When to Use This Service

Use this service when:

  • You discover physical damage to buildings, rooms, equipment, or other university property.
  • You were involved in an incident (e.g., bumping a vehicle, breaking furniture) and want Campus Safety/Facilities to be aware.

Do not use this service when:

  • There is active fire, explosion, major structural damage, or other emergency → Call Campus Safety / 911 first.
  • Personal (non‑university) property is damaged or stolen without any damage to university property those may be logged under different categories (e.g., theft, lost property) depending on your internal rules.
