What Is It?
Use this form to report a possible violation of the Student Code of Conduct or other concerning student behavior. This form can be used by students, faculty, staff, and others who wish to report alleged misconduct or behavior that may violate university policy.
Examples include:
- Alcohol or drug violations
- Disruptive or disorderly conduct
- Harassment, threats, or intimidation
- Vandalism or damage to property
- Failure to comply with the directions of university officials
- Other behavior you believe may violate the Student Code of Conduct
If you need to report an emergency or an immediate threat to safety, DO NOT use this form. Call Campus Safety / University Police at 610-409-3333 or 911 instead.
After you submit this form:
- The report will be reviewed by Student Affairs / Student Conduct staff (and Campus Safety as appropriate).
- Staff will determine the next steps under the conduct process, which may include additional information‑gathering, outreach to impacted parties, and possible conduct proceedings.
- Reports related to students will be recorded in the university’s conduct case‑management system (e.g., Maxient) and handled according to due‑process standards.
You may submit anonymously; however, providing your name and contact information can help staff follow up with you if they need clarification and may be important if you are a witness in a conduct matter.

- Campus Safety and Residence Life staff entering reports after incidents
When to Use This Service
Use this service when:
- You witness or become aware of behavior that may violate the Student Code of Conduct.
- You want Student Affairs to be aware of an incident even if you are not sure whether it is a policy violation.
Do not use this service when:
- There is an active emergency or serious, immediate threat → Call Campus Safety / 911.
- You are reporting a crime but not a conduct issue; in most cases, Campus Safety should be contacted directly and will create the appropriate incident record as well
