Title IX – Report Sexual Misconduct or Sex‑Based Discrimination

What Is It?

Use this form to report sexual misconduct or sex‑based discrimination so that the Title IX office can review the information, provide support, and determine the appropriate process for response.

You can use this form to report:

  • Sexual assault
  • Dating or domestic violence
  • Stalking
  • Sexual harassment, including hostile‑environment conduct
  • Sex or gender‑based discrimination that affects access to programs, activities, or work
  • Other conduct you believe may fall under the college’s Sexual Misconduct / Title IX policy or related non‑discrimination policies

Some reports will fall under Title IX regulations (e.g., defined offenses like sexual assault, dating/domestic violence, domestic violence, stalking, and certain sexual harassment occurring in the institution’s education program or activity within the United States) and will be resolved through the Title IX grievance process. Others may be addressed under other policies, such as the Student Code of Conduct or employee policies, but all are taken seriously and reviewed by appropriate staff.

After you submit this form:

  • The Title IX Coordinator (or designee) will review your report, provide required notices, and reach out as appropriate to discuss:
    • Safety and supportive measures (no‑contact orders, academic or housing adjustments, workplace changes, etc.),
    • Options for informal or formal complaints,
    • Connections to advocacy, counseling, and other support resources.
  • The report may also be shared, on a limited basis, with Campus Safety, Student Affairs, HR, or other offices as needed to protect individuals and the campus community.

You may report on your own behalf or on behalf of someone else. You can provide your name and contact information or, where permitted, report anonymously. Providing contact information allows the Title IX office to follow up with you about safety, options, and resources.

This form is not a substitute for emergency response.
If you or someone else is in immediate danger or needs urgent medical care, call Campus Safety at 610-409-3333 or 911 first, then submit this form if appropriate.

Who Is the Primary Audience?

  • All students, faculty, and staff
  • Campus Safety, Student Affairs, HR, and other “responsible employees” to report incidents brought to their attention.

How this differs from other services

  • Different from “Sexual Assault – Internal Incident Record”

    • That service is primarily an internal, operational log for Campus Safety/Title IX/Student Affairs once a sexual assault case is already being handled.
    • The Title IX service is the front‑door reporting channel for any sexual misconduct/sex‑based discrimination, whether or not it’s clearly sexual assault.
  • Different from “Student Conduct Incident Report”

    • Student Conduct handles a broad range of policy violations.
    • Title IX is specifically about sexual misconduct and sex‑based discrimination and triggers different notice, supportive measures, and grievance requirements under federal regulations.
    • A single behavior might be processed as “Sexual Misconduct (Title IX)” or “Sexual Misconduct (non‑Title IX)” depending on definitions and jurisdiction, but the Title IX Coordinator still reviews all such reports.
